A car maintenance log you can print or open in a spreadsheet.

Start with a simple record: date, odometer, work, status, cost, and the evidence you kept. Use the blank form below or download a CSV file for Excel or Google Sheets.

What each row should answer

Choose the exact vehicle first, then make one row for each distinct service action or inspection. Record the date and mileage, what was actually done, its status, any cost you want to track, and the location of the invoice or receipt. If a shop invoice covers several actions, you can cite that same invoice in several rows.

Be precise: “Brake pads inspected” and “brake pads replaced” are different records. If an earlier service cannot be verified, leave its date and mileage blank and mark the history unknown.

Printable maintenance log

Write the vehicle year, make, model, and VIN on the sheet. Then print this page, or use your browser’s “Save as PDF” option. The blank rows are intended for pen entries.

How to use the CSV

The download contains column names and one clearly labeled example row. Delete the example before recording your own car. Make a copy per vehicle or keep the vehicle label and VIN columns when combining several cars. Spreadsheet programs can open a CSV; dates and currency formatting may vary by program.

Oil and filter replaced

2026-09-15 · 62,410 miles · completed · receipt “Shop invoice 123.” This demonstrates what belongs in a row; it is not a recommended interval.

Turn a log into a plan

A log tells you what happened. The manufacturer’s schedule tells you what to check next. Read our records guide for evidence and our mileage and time guide for the next deadline. MaintainAhead combines those records with new odometer readings to help plan upcoming work.
